Assessment

Intake evaluation – 90 minutes, $300

  • Parents only, usually virtual

  • Will allow us to discuss your concerns and observations, ask clinical questions, and get a developmental history on your child to inform later diagnosis

Psychoeducational evaluation - $3250

  • When you’re trying to determine the root cause of your child’s difficulties and want to rule out possible effects of learning disability, ADHD, mood disorder, etc.

  • Includes: clinical interview; two 3-hour sessions evaluating cognitive abilities, achievement skills, memory and attention, fine motor skills, adaptive skills, and parent/teacher/self-report forms on attention, behavior, and mood symptoms; feedback session; full report

  • Adding an autism rule-out = $3750 total

ADHD evaluation (diagnosis only) - $500

  • For children and adults

  • Includes: parent and teacher report forms, feedback session, and abbreviated report

ADHD evaluation (diagnosis and treatment planning) - $2500

  • For children and adults

  • When academics are on-target and ADHD/behavior/mood is the main concern

  • Includes: clinical interview, 3-hour testing session to assess cognitive abilities, memory, and attention, as well as parent/teacher/self-report forms on attention, executive functioning, behavior, and mood symptoms; feedback session; full report

ASD evaluation (ages 18m+) - $2500

  • For initial diagnosis or later re-evaluation of symptoms and progress

  • Includes: clinical interview; one 3-hour session assessing cognitive/developmental ability, autism symptoms through the ADOS-II, adaptive skills, and parent/teacher/self-report forms on attention, behavior, and mood symptoms; feedback session and full report

Early admittance evaluation - $850

  • For those requesting early admittance to a different school grade

  • Includes: brief cognitive, brief achievement, parent and teacher report forms, feedback session, and abbreviated report

IQ or Achievement only - $850

  • Commonly used to support a request for gifted services or to update a needs assessment for school/college accommodations

  • Includes: cognitive or achievement assessment and appropriate parent/teacher/self report form

Targeted evaluation – $250 hourly estimate depending on time

  • For when there are additional diagnostic questions that have not been answered in previous evaluations or treatment 

  • Tests determined based on clinical question

Payment

Payment is due up front at the time of service.  You may pay by check, credit card, or Zelle transfer.  While we cover the 3% service charge on credit card payments for therapy, we request payment through check or Zelle for assessments.

We will also provide you with invoices and superbills to submit to your insurance company for reimbursement of Out of Network benefits.


 Good Faith Estimate 

You have the right to receive a Good Faith Estimate (GFE) explaining how much your medical care will cost.  At our office, this means: 

  • Under the law, health care providers are required to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.  This includes a total expected cost of any non-emergency items or services such as medical tests, prescription drugs, equipment, and hospital fees.

  • You should receive your GFE in writing at least 1 business day before your medical service, item, or appointment.  You can also ask your health care provider for a GFE before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.

  • A copy of your GFE will be saved in your client portal.
